Care Navigator Partnership
Patients over the age of 16 years can now access a care navigation service. This free service is delivered in partnership by Community Agents Essex, Age UK Essex and the Provide Essex Lifestyle Service. If you are struggling with a non-clinical issue and it is impacting on your health and wellbeing - you can self refer to the service.

You can now book your pregnancy directly with the Midwife as you no longer need to see the GP.

Antenatal screening for sickle cell disease and thalassaemia aims to offer informed reproductive choice by identifying parents who are genetic carriers and at risk of having a baby that inherits the condition. The standard recommendation is that the sickle cell and thalassaemia screening test should be done between 8-10 weeks. Early screening is critical to this choice, including the option to terminate pregnancy in the first trimester. All screening tests and decision making should be completed by 12-16 weeks of gestation.
Women are advised to contact their midwives as soon as possible to arrange for their booking bloods to be done or self-refer for screening to:

Shared Decision Making
Shared decision making (SDM) is the conversation that happens between a patient and their health professional to reach a healthcare choice together. This conversation needs patients and professionals to understand what is important to the other person when choosing a treatment.

This is a national initiative allowing you to give your feedback on the service your GP practice provides.

Evening and Weekend GP Service
We are very pleased to be offering weekend appointments to our patients.
These appointments are available at a local health centre and can be booked through our receptionists.
We are encouraging people to use this service which provides additional appointments to fit in with your busy lifestyles.
Patients can see a GP, healthcare assistant or nurse for routine appointments, health checks and treatments. People with long term health conditions such as asthma and diabetes can also access these appointments.

If you have booked an evening or weekend appointment in West Essex via your GP surgery and need to cancel or change it, please ring NHS111 and select option 3.
We have introduced an online system to enable you to cancel appointments; book appointments with your usual GP; order your repeat prescription; or view a brief summary of your medical record.

Changes to the way you collect your prescription
The NHS is introducing the Electronic Prescription Service. This could make collecting prescriptions easier and more convenient for you.
This service gives you the option to ‘nominate’ the place you choose to get your medication from so that they can receive your prescriptions electronically. This could save you a trip to your GP practice just to collect a prescription — particularly if you have regular prescriptions.
