New Patient Registration

You will need to come to the surgery to register. If you have your existing medical card with details of your previous doctor, please bring it with you. If you have lost or misplaced it, don't worry, we will still be able to register you.

Please note that if you wish to register with the Practice call and book an appointment with our Registration Team.

Don't forget to bring some proof of your current address and, if not a UK national, your passport and any other documents which prove your eligibility to receive NHS services. Remember, the onus is on YOU to prove your eligibility to receive NHS treatment.

You will be asked to complete a questionnaire about your medical history to assist us. You will be offered a new patient health check (at a later date). If you are taking repeat medication from your previous doctor, it will assist us if you can bring your repeat medication order form with you.
