Travel Vaccinationsglobal air travel

Our Practice Nurses run travel advice clinics, not only to provide the appropriate vaccinations but to give practical advice on travelling abroad. Appointments should be booked at least 4 weeks prior to travel in order to achieve the appropriate level of immunity.

Patients registered at the Lister Medical Centre may download this form, print and complete a copy for each member of the family travelling, and return to the Lister Medical Centre at least 6 weeks prior to travel. You will need to contact the surgery 2 working days after we receive it to find out which vaccinations are required and if malaria tablets are needed. You will be advised how soon you need to make an appointment with one of the Practice Nurses.
